PIERCINGS FOR MINORS

Requirements

Due to COVID-19 we will not be accepting booking for anyone under the age of 13.
Anyone under the age of 18 must be accompanied by a parent or legal guardian. All minors must provide identification and proof of age and relationship to their guardian. Jewelry change-outs are excluded from this rule. Acceptable identification and proof of age and relationship include the following:
All forms of acceptable identification are listed under the adults section. Names for both parent and minor on the last names or addresses must match.
If the parent has a different last name and/or address than the minor the following is required. Legal documentation (birth certificate, marriage certificate, divorce papers, adoption papers, etc.) must be provided to verify the relationship.
If the legal guardian is not a parent, then the adoption papers must be provided to verify the guardian’s legal right to sign for the minor.
All legal documentation (birth certificate, marriage certificate, divorce papers, adoption papers, etc.) must be notarized.
A school issued photo ID or yearbook picture accompanied by a birth certificate is acceptable identification for minors younger than driving age.
